Where do the WHO 2008 waist to hip thresholds come from?
The cut points come from the WHO Expert Consultation on Waist Circumference and Waist-Hip Ratio held in Geneva in December 2008 (report published 2011). The panel reviewed prospective cohort data linking central adiposity to type 2 diabetes, cardiovascular disease, and all-cause mortality, and set substantially increased risk at a WHR of 0.90 or above for men and 0.85 or above for women. This calculator splits the at-risk zone further into a moderate band (0.90 to under 1.00 for men, 0.85 to under 0.90 for women) and a high band (1.00 or above for men, 0.90 or above for women), which mirrors common clinical and public health practice.
Why is waist to hip ratio a better cardiovascular risk predictor than BMI?
BMI uses total weight and height and cannot tell where fat sits. Two people at the same BMI can have very different cardiovascular risk profiles if one carries fat around the abdomen (visceral, metabolically active) and the other carries it around the hips and thighs (gluteofemoral, much less active). The INTERHEART study (Yusuf et al., Lancet 2005) showed WHR was a stronger predictor of myocardial infarction than BMI across 52 countries, and later meta analyses have repeated the finding for type 2 diabetes and overall mortality. WHR is a more direct readout of the body fat that actually drives risk.
What is waist to height ratio (WHtR) and how is it different from WHR?
Waist-to-height ratio is waist circumference divided by height in the same unit. Unlike WHR, it does not use hip circumference. Keeping waist below half of height is a widely used screening rule, but 0.5 is not a universal diagnostic cutoff for every age or population. The calculator shows WHtR alongside WHR as another screening signal, not as a diagnosis.
What is the difference between an apple shape and a pear shape?
Apple shaped bodies carry more fat around the waist and abdomen, which pushes the WHR up. Pear shaped bodies carry more fat around the hips and thighs, which keeps the WHR down. The distinction matters because abdominal (visceral) fat surrounds the liver and other internal organs and is highly active metabolically. It releases free fatty acids and inflammatory signals that drive insulin resistance, dyslipidemia, and hypertension. Gluteofemoral fat does not carry the same risk and may even be mildly protective for cardiometabolic outcomes. At the same BMI an apple body shape carries meaningfully more cardiovascular and diabetes risk than a pear body shape.
Where exactly should I measure my waist and my hips?
For the waist, the WHO 2008 protocol is to measure at the midpoint between the lowest rib and the top of the hipbone (iliac crest), or simply at the top of the iliac crest if the midpoint is hard to find. Stand relaxed, breathe out normally, and place a soft tape level all the way around the abdomen without compressing the skin. For the hips, wrap the tape around the widest point of the buttocks, again keeping it level. Take each measurement twice and use the average. Consistent landmarking matters more than the absolute number; small placement differences can change a result by several centimeters.
