What is the Revised Cardiac Risk Index (RCRI)?
The Revised Cardiac Risk Index (RCRI), also called the Lee Index, estimates the risk of major cardiac complications in the 30 days after major noncardiac surgery. Thomas Lee and colleagues published it in 1999 after studying more than 4,000 surgical patients. It uses six simple yes or no risk factors, so you can score it at the bedside without a complex model. A higher count of factors moves the patient into a higher risk class.
How is the RCRI score calculated?
Each of the six Lee risk factors that applies adds 1 point, and the total is the sum (an integer from 0 to 6). The six factors are high-risk surgery, a history of ischemic heart disease, a history of congestive heart failure, a history of stroke or transient ischemic attack, preoperative insulin treatment, and a preoperative serum creatinine above 2 mg/dL. What does my RCRI class mean for cardiac risk?
The point total maps to four risk classes. In the original Lee 1999 validation cohort, Class I (0 points) carried about a 0.4% rate of major cardiac complications, Class II (1 point) about 0.9%, Class III (2 points) about 6.6%, and Class IV (3 or more points) about 11%. Major complications here mean myocardial infarction, pulmonary edema, ventricular fibrillation or primary cardiac arrest, and complete heart block. More recent validations report somewhat higher event rates, so treat these figures as the classic estimates rather than exact modern probabilities.
What counts as high-risk surgery in the RCRI?
In the RCRI, high-risk surgery means an intraperitoneal procedure, an intrathoracic procedure, or a suprainguinal vascular procedure (such as aortic or other major vascular surgery above the groin). These operations carried the largest cardiac stress in the original study, so they score 1 point. Lower-risk operations, such as endoscopic, superficial, cataract, or ambulatory procedures, do not score this factor. Why does the calculator ask for a creatinine value instead of yes or no?
The renal risk factor is defined as a preoperative serum creatinine above 2.0 mg/dL, which is about 176.8 micromol/L. Entering the actual lab value lets the tool apply that threshold for you, so you do not have to remember the exact cutoff. A creatinine of exactly 2.0 does not score the point, while any value above it adds 1 point. If your lab reports creatinine in micromol/L, divide by 88.4 to convert to mg/dL before entering it.
