How do you calculate an odds ratio from a 2x2 table?
Label the table cells a (exposed cases), b (exposed controls), c (unexposed cases), and d (unexposed controls). The odds ratio is the cross-product ratio, (a times d) divided by (b times c). It compares the odds of the outcome in the exposed group with the odds in the unexposed group.
What does the odds ratio value mean?
An odds ratio of 1 means the odds of the outcome are the same in both groups. A value above 1 means the odds are higher in the exposed group, and a value below 1 means they are lower. For example, an odds ratio of 2 means the odds of the outcome are about twice as high in the exposed group.
How is the confidence interval computed?
This calculator uses the Woolf logit method. It takes the natural log of the odds ratio, finds the standard error as the square root of (1/a + 1/b + 1/c + 1/d), then builds the interval as exp(ln(OR) plus or minus z times the standard error). The z value is 1.64 for 90 percent, 1.96 for 95 percent, and 2.58 for 99 percent.
When is the odds ratio statistically significant?
An association is usually called statistically significant at a given level when the confidence interval does not include 1. If the interval runs from below 1 to above 1, the result is consistent with no association at that confidence level. A wider interval reflects more uncertainty, often from small cell counts.
What happens when one of the cells is zero?
A zero cell makes the odds ratio either 0 or undefined and the standard error infinite. To keep the result usable, the calculator adds 0.5 to all four cells (the Haldane-Anscombe continuity correction) whenever any cell is zero. This is the standard fix reported by epidemiology software, and it slightly pulls the estimate toward 1.
