Where does the Body Roundness Index come from?
Diana Thomas and colleagues introduced BRI in Obesity in 2013 as a single shape index that would capture how round a body is, independent of total weight. It models the human body as an ellipse whose semi-major axis is height and whose semi-minor axis is derived from waist circumference, then reports the eccentricity of that ellipse on a one-number scale that typically lands between 1 and 9 for adults. The Thomas-Bredlau group refined the index against the 1999 to 2018 NHANES cohort in a 2024 update, which is where the five risk bands this calculator uses (3.41, 4.45, 5.46, 6.91) come from. Earlier work used continuous mortality curves; the 2024 update gave clinicians and consumers a clean banding aligned with hazard ratios.
Why does BRI beat BMI for visceral fat?
BMI uses only weight and height, so it cannot tell a lean muscular adult apart from a sedentary adult with high abdominal fat at the same weight and height. BRI replaces the weight input with waist circumference, the body part where visceral (organ-surrounding) fat actually accumulates. Visceral fat is metabolically active in a way subcutaneous fat is not, and it is the form most strongly linked to insulin resistance, fatty liver, and cardiovascular disease. NHANES analyses consistently find BRI is a better single predictor than BMI for type 2 diabetes, coronary artery disease, and 10-year all-cause mortality, especially for the apple-shaped pattern that BMI misses entirely.
How does the ellipse model actually work?
The formula treats the body as a vertical ellipse. The semi-major axis is half your standing height, so a 175 cm tall adult has a 0.875 m vertical radius. The semi-minor axis is derived from waist by dividing waist circumference by 2 times pi, the same way you would back out a radius from a circumference. The ratio of those two radii gives the eccentricity of the ellipse, and the constants 364.2 and 365.5 in the formula rescale that eccentricity to a 1 to 20 range that lines up with the NHANES distribution. Rounder bodies (waist closer to height/pi) score higher. The math breaks down only when waist exceeds pi times height, which never happens anatomically, so the model stays well defined across every realistic input.
What does a BRI of 6.91 or higher mean?
The calculator uses 6.91 as its top BRI band cutoff from a population analysis. An observational hazard band does not determine an individual's cause of risk or amount of visceral fat. A high BRI is not a diagnosis; discuss waist circumference, blood pressure, lipids, glucose, and other risk factors with a clinician.
Do ACSM and the AHA recommend BRI over BMI?
Not yet, but the direction of travel is clear. The American College of Sports Medicine and American Heart Association both still list BMI as the primary screening tool in their published guidelines (ACSM Guidelines for Exercise Testing and Prescription, AHA scientific statements on obesity and cardiovascular risk), but both also recommend pairing BMI with waist circumference rather than relying on BMI alone, and both cite the documented BMI gaps for athletes and for visceral-fat phenotypes. BRI is increasingly cited in cardiovascular epidemiology papers (Thomas-Bredlau 2024 and follow-ups in JAMA Network Open and the European Heart Journal) as a candidate replacement or complement, but it has not yet replaced BMI in any major society guideline. Treat the BRI band here as research-backed context alongside BMI and waist, not as a substitute for a clinical assessment.
